We have a new messaging service which is a quick, simple and confidential way for you to contact us about a medical, administrative or prescription issue.

Answer a few short questions and we will get back to you within 2 working days. Please do not use this online form for urgent or emergency requests.

The completed forms can only be actioned during opening hours. Therefore the forms will not be read between the hours of 5.00pm to 8.00am, on the weekends (Saturdays and Sundays) or bank holidays. Please contact 111 during out of hours or 999 if this is an emergency.
